Whether you require assistance with daily activities, personal care, meal preparation, showering, domestic cleaning, or simply want to get out for shopping, outings, and social participation, we are here to support you at every step. Our services also include skills development, support coordination, and Supported Independent Living (SIL) to help you achieve independence with confidence.

SIL is a type of support or supervision involving daily tasks to help people live independently while building their skills. It includes activities, such as having a person to help with personal care tasks or cooking meals. SIL can also help build these skills so that one can be more independent. Your Community Care Services manages group homes in Canberra, located in different parts of the ACT, VIC, SA and NT. These group homes are supervised by well-trained team leaders and house managers to oversee the complex needs of participants with intellectual disabilities, physical disabilities, autism and Down syndrome. Our team of trained community nurses manages catheter management, medication administration, wound management, acquired brain injury and other supports depending on the need.
Providing accessible public or community transport for people with disabilities can reduce their need to use taxis and the associated costs of taxi vouchers. The design of shopping centres, and access to public or community transport, enable people with disabilities who can use public transport to be independent and shop for themselves. They do not have to rely on others to shop on their behalf. Providing orientation and mobility services to people with moderate vision impairment can reduce their likelihood of falling or accidents, leading to further disability or impairment. Creating better networks and breaking down stereotypes to promote economic and social participation.
We provide NDIS participants with personal care support in the following areas: Showering, grooming, dressing, toileting, hygiene. Nursing: medications, prescriptions, injury management, wound care, hoisting/lifting assistance. Housework: cleaning, laundry, meals, shopping, and paying bills. Transport: Medical appointments, visiting friends and family, social events.
The
development of daily living and life skills focuses on training and development
activities undertaken by customers or their caregivers to increase their
autonomous ability.

We provide services to assist with tasks at home such as home maintenance, house cleaning, laundry and gardening. These include washing dishes or tidying your living spaces to organise key areas in your home. We can assist with your cleaning and organising laundry needs. In addition, we will happily water your plants, add much-needed flowers and even ensure that you have a neat lawn. We consider your concerns regarding outside areas.
The NDIS can fund transport in four main ways: paying for taxis or rideshares if you cannot catch public transport, paying a support worker to drive you or travel with you, building your capacity to get around and funding modifications to your vehicle.
The NDIS describes assistance with accommodation and tenancy obligation as support to guide, prompt, or undertake activities to ensure the participant obtains/retains appropriate accommodation. This may include assistance in applying for rental tenancy or undertaking tenancy obligations.
Assisting participants in group-based community, social and recreational activities. These include various social activities, such as singing and music, arts and craft, chess, playing cards, disco and day trips, job skills and money handling.
The role of a support coordinator is to connect participants with an NDIS plan for support and services in the community. Our role as support coordination is to complement your life and help implement your NDIS plan. This role will allow your support coordinator to manage these resources effectively and obtain the best outcomes from your plan.
Community health nurses work with NDIS participants to help prevent diseases, maintain health and treat existing health problems. Community nursing aims to support and maintain patient independence, safety and a healthy lifestyle while providing caregivers with assistance. Our staff is well trained and fully insured with appropriate insurance, including workers’ compensation and public liability. Other staff certifications include certificates III and IV for disability, first aid, working with children/working with vulnerable people’s checks/workers’ screening, driver’s licence and comprehensive insurance. We are pleased to work with other NDIS providers in the support areas mentioned above.
